Health Support Care is founded and run by Registered Nurses with experience across acute nursing, aged care, disability support, gastroenterology, endoscopy, dysphagia care and complex home nursing. Our services fall into three areas: complex nursing care, disability support, and aged care. No matter the level of support you need, it’s delivered by a team led by Registered Nurses, who genuinely care.
For participants with high-intensity health needs, our Registered Nurses bring hospital-level clinical skill into your home. With backgrounds in gastroenterology, endoscopy and dysphagia care, complex care is what we know best, and we deliver it with warmth, patience and respect.
Great disability support is about more than help with daily tasks. It’s about building skills, confidence and independence, your way. We support participants of all ages and abilities, always at your pace and around your goals.
We help older Australians stay safe, comfortable and connected at home. Our nurses bring five years of aged care experience and a genuine soft spot for this work, including specialised support for people living with dementia.

We welcome referrals from support coordinators, plan managers, hospitals and families. Send through the referral form and our nurse-led team will be in touch to discuss the participant’s support needs.
