We know that starting with a new provider can feel like a big decision, especially when health needs are complex. Our team takes the time to understand the participant’s routines, risks, preferences and clinical requirements before support begins. From there, we can work with participants, families, support coordinators, plan managers, case managers and health professionals to help create a safe and practical care arrangement.

Can Health Support Care support participants with both disability and complex nursing needs?

Yes. Health Support Care is led by Registered Nurses, which means we can support participants who need practical disability support as well as more involved clinical care. This may include help with personal care, daily routines, medication management, feeding tube care, dementia support, brain or spinal injury care, and other high-care needs.

We start by learning about the participant’s current health needs, daily routine, risks, goals and preferences. Where needed, we may also review care plans, hospital discharge information, medication requirements, feeding tube needs, mobility concerns and input from families or other health professionals. This helps us understand what level of support is required and how care can be delivered safely at home.

Medication support is handled carefully and in line with the participant’s needs, plan and clinical requirements. Our nursing background allows us to take a considered approach, especially when a participant has multiple medications, changing health needs or additional risks that need to be monitored.

We believe good care relies on clear communication. Health Support Care can work alongside support coordinators, families, plan managers, case managers, GPs, hospitals and allied health professionals to help make sure support is consistent and properly understood. This is especially important when a participant has complex care needs, recent hospital involvement or a change in their health.

We take things slowly and respectfully. A new person coming into the home can feel overwhelming, so we focus on building trust, explaining what is happening and giving the participant as much choice and control as possible. Our team takes the time to understand how the person likes things done, what helps them feel comfortable and what may make support feel easier.

Yes. We can support older people and participants living with dementia who need help with daily care, routines, personal care and nursing-related needs. Our approach is calm, respectful and practical, with care shaped around familiarity, safety and the person’s usual way of doing things.

Safety starts with the right clinical knowledge. Health Support Care is run by Registered Nurses with experience in high-care needs, including feeding tube care, medication management, dysphagia care, tracheostomy-related care and ventilator-dependent support. We take the time to understand each participant’s care requirements, follow appropriate plans and monitor for changes that may need further clinical attention.

Yes. Families often know support is needed but are not always sure what that should look like. If you contact Health Support Care, we can talk through the person’s current situation, what is becoming difficult, any health concerns and what support may be suitable under NDIS, Aged Care or DVA arrangements.

We ask the right questions before support begins. This includes how the participant likes their day to run, what they can do independently, what they need help with, how personal care should be approached, how meals or medications are managed, and what makes them feel safe and respected. From there, we shape support around the person’s routine instead of expecting them to fit into ours.
