We bring hospital-level knowledge into home and community settings, helping participants and clients feel safer, more supported and better understood in their everyday care.
Health Support Care is led by Registered Nurses with more than eight years of experience in general acute nursing care, anaesthetic care, and pre- and post-operative support, as well as five years of direct experience in aged and disability care.
This background gives our team a strong understanding of both clinical care and everyday support. We know that good care is not only about completing tasks. It is about recognising changes, understanding risk, communicating clearly and helping people feel comfortable with the support they receive at home.
Our specialist experience includes gastroenterology, endoscopy, bronchoscopy, dysphagia care, feeding tube care and airway-related support. We can support participants and clients with Percutaneous Endoscopic Gastrostomy (PEG), Percutaneous Endoscopic Gastrostomy with Jejunal extension (PEG-J), Nasogastric tube (NGT) and Nasojejunal tube (NJT) care, as well as people living with brain injury, spinal injury, dementia, autism, intellectual disability, mental health needs, physical disability and sensory disability.
Health Support Care also has experience supporting people with tracheostomy care, laryngectomy and pharyngectomy-related needs, ventilator-dependent care, and pre- and post-feeding tube insertion care. This means we can step in when support requires more than general assistance and needs a confident, clinically informed approach.
Many people with complex health needs spend a lot of time moving between hospitals, specialists, appointments and home. Our role is to help make care at home feel more coordinated, less stressful and more personal.
We take the time to understand each person’s routine, health requirements, communication style, risks, preferences and goals, then shape support around what is safe and workable for them. Whether someone needs help after surgery, support with medication, feeding tube care, dementia care or high-care disability support, our focus is always on care that feels calm, respectful and properly managed.
Health Support Care was founded by Registered Nurses who believe great care starts with the right values, lived out in every visit, every shift, every day.
Whether you’re a participant, a family member or a support coordinator, we’re happy to talk through how we can help. Call, email or send us a message and we’ll get back to you as soon as we can.
We welcome referrals from support coordinators, plan managers, hospitals and families. Send through the referral form and our nurse-led team will be in touch to discuss the participant’s support needs.
